By submitting this form you are requesting the PTMAA perform a record search of active and abanoned tank sites for the address(es) provided and agree to pay all related fees within thirty (30) days of invoice.

Fees:

  • $20.00 + GST per address (consists of one municipal address and its associated lot/block/plan and legal land description)
  • $10.00 + GST per report (up to three reports may be available: Site/Tank Detail, Site Diagram and Tank Closure Report)

Reports will automatically be sent unless you request they be excluded in the comment section of this form.
